General Summary

The Client Linkage and Retention Facilitators will support HTC activities through linkage and Peer counselling of HIV positive clients at the facility where they will be placed. Their major roles include; health education, linkage of HIV positive clients to care, follows up of clients and preparing monthly reports.

Key Responsibilities

  • Support targeted risk-based HIV testing at different care entry points of the facility 
  • Support formation of family Support Groups or Peer support groups (adolescents and discordant couples). 
  • Support health workers to prepare for client visits 
  • Support Linkage and retention of HIV positive clients identified at all care points including outreaches, TB care point, teenage centers, VCT/PICT to receive HIV care. 
  • Provide support in the management of non-suppressed clients. 
  • Follow up of clients who miss visits by more than a week using telephone or home visits if they live within your area of residence.  
  • Link up with the other community resources to follow up clients e.g. through VHT or community based organizations. 
  • Support documentation and reporting of activities.  
  • Any other duties that may be assigned by the supervisor
  •  Any other duties as assigned by the supervisor

Person Specification

  • Ordinary level certificate 
  • Basic training in HIV/AIDS 
  • Possess basic Peer Counselling skills 
  • Fluent in English and local language
  •  Living with in the health facility catchment

The District HIV prevention and Key Population Program Officer will provide technical assistance to field office and HF staff and DHTs to ensure timely attainment and sustenance of quality HIV/AIDS prevention services especially those targeting key and priority populations
